Social Worker Responsibilities
Social workers visit clients in their homes and provide initial evaluations of the physical, psychosocial, and environmental needs of clients and their families. Social workers help to develop appropriate and individual care plans based on perceptions and given feedback. Social workers may counsel clients and their families regarding chronic condition management and evaluate clients’ responses to care provided.
Social workers also act as the care coordinator for assigned individuals and their families, initiating referrals as necessary and supervising and directing caregivers regarding the care of our clients. When needed, social workers collaborate with a clients’ other healthcare professionals to make sure that all health needs are being considered.
In many cases, clients are regularly scheduled for doctor’s appointments due to chronic conditions, so social workers make sure to add transportation to medical appointments and follow-up visits into regular care schedules.
