About ComForCare – Senior Home Care Services in Jacksonville, Florida
ComForCare Home Care is a premier provider of private duty, non-medical home health care in Jacksonville, Florida. Our specialty is providing excellent care to our senior population. Our caregivers provide personal care for individuals seeking assistance with ADL’s (Activities of Daily Living). We provide these services in many settings including residences, assisted living facilities, independent living facilities, nursing homes, hospitals or wherever our clients reside. We offer a number of life improving services such as elderly companionship and Alzheimer’s care. Our goal is to employ and provide our clients with the highest quality caregivers available.
We serve Jacksonville area clients who are 18 years of age or older with particular focus on the senior community in Duval, Clay, Nassau and St. Johns Counties. Many individuals prefer to stay in their homes rather than go to an assisted living community or a nursing home. There are many adults who are not able to care for their own daily needs and this is what makes our senior home care services in Jacksonville so vital.
ComForCare Home Care was established in 1996 as a premier provider of private duty, non-medical home care with over 190 branches spanning the entire United States. ComForcare Home Care of Jacksonville has earned the reputation as a premier and professional in-home health care provider. This has been accomplished by our ability to provide an unparalleled quality of care.
ComForCare maintains extremely high and specific standards regarding the in-home health care that we provide. We are licensed by the State of Florida, Agency for Health Care Administration (AHCA) with a Home Health Agency and Home Maker and Companion license. We are meticulous in our selection of the most highly qualified, responsible and compassionate caregivers. We only employ home health caregivers whom we feel comfortable sending into our own parent’s homes.
The In-Home Health Care Process
The ComForCare Home Care Admission Service Agreement is completed to begin the application process. A registered nurse from our agency then meets with the prospective client/family to write a comprehensive assessment that outlines the care plan and specific client needs. A copy of the assessment stays in the client’s home and serves as a guide for the ComForCare caregivers. The registered nurse (RN) provides case management by staying in touch with the client, the responsible party, and the caregivers. The RN makes regular home visits to the client to reassess needs and levels of care.
