Caregiver Position - Full Position Description
Home Health Aide (HHA) / Certified Nurse Assistant (CNA) / Caregiver (A person who was formerly HHA or C.N.A. certified and worked for a minimum of 1 year).  

  1. One or more of the following criteria: One year minimum of continuous experience as a Home Health Aide or Nurse Aide in a health care environment; Completion of a State (or similarly accredited) Home Health Aide or Nurse Aide training program; Completion of one year of classroom and clinical training from an accredited nursing program, medical assistance program, or other applicable direct care training program.
  2. Satisfactory compliance with ComForcare bonding criteria.
  3. Demonstrated ability to effectively communicate and interact with clients, families, coworkers, and all supervisors.
  4. Ability to accurately follow and process written and verbal instructions.
  5. Satisfactorily demonstrate and perform competency in clinical skills through competency checklist and skills demonstration test.
  6. Minimum score of 80% on competency exam (NLN Test). 

Companion Position - Full Position Description

The Companion provides specifically defined, non-skilled health care services under the supervision of a Registered Nurse or Licensed Practical Nurse to enable the client to remain in the home.  The Companion is a non-licensed health care employee who provides personal care and light housekeeping services in the home setting of clients and families who are unable to perform these activities independently.

  1. Current Indiana Driver’s License, Proof of automobile insurance,  and daily availability of a car for business use.
  2. Satisfactory compliance with ComForcare bonding criteria.
  3. Demonstrated ability to effectively communicate and interact with clients, families, coworkers, and all supervisors. Including ability to proficiently follow and process written and verbal instructions in the English language.
  4. Satisfactorily demonstrate and perform competency in direct care skills through completion of competency checklist.

     

H.H.A. Position Description - Full Position Descripition

  2. Active home health aide license in good standing
  3. Satisfactory compliance with ComForcare bonding criteria.
  4. Demonstrated ability to effectively communicate and interact with clients, families, coworkers, and all supervisors.
  5. Ability to accurately follow and process written and verbal instructions.
  6. Satisfactorily demonstrate and perform competency in clinical skills through competency checklist and skills demonstration test.
  7. Minimum score of 80% on competency exam (NLN Test).

Licensed Practical Nurse (LPN) - Full position Description
Provide nursing care to client in the home setting.  Observe and assess the client and/or caregiver to enhance the quality of life.  Demonstrate individualized creativity in educating the client and/or caregiver.  Follow nursing policy and procedure per agency standards.  Follow the plan of care according to physician orders.  Demonstrate understanding of federal and state regulations and accreditation standards.

  1. Licensed Practical Nurse.
  2. Minimum of 2 years’ experience in a healthcare setting.
  3. Excellent oral and communication skills.
  4. NAPNES certification.
  5. Possess a current, unencumbered license to practice nursing within the state of Indiana.

Registered NurseFull Position Description

Demonstrate proficient skills using assessments to admit, transfer, reassess, and discharge home health clients based on assessed needs. Coordinate and supervise LPNs, and other caregivers in the delivery of client care. Maintain compliance with agency policy and procedures.  Follow federal and state regulations and accreditation standards.

  1. Be a registered nurse (R.N.) with a current license.
  2. Minimum of one year of nursing experience as a R.N.
  3. Work positively and favorably with clients, families, and staff.
  4. Possess a current, unencumbered license to practice nursing within the state in which candidate is applying for work.
