The Transitions of Care Program through ComForCare Home Care provides clients with one-time care assistance or develops and updates, as needed, care plans. Our nurses are able to evaluate needs and follow a client’s discharge plan. This can include developing a personalized routine based on the plan provided and conducting home safety checks as needed, which helps ensure a safe living environment.
