Medicare’s Breakthrough Program for Dementia Care: A New Era in Dementia Home Care Services

Participants gain access to a full spectrum of services, including:
- Comprehensive Assessments: Care Navigators conduct in-depth evaluations to create personalized care plans.
- Home Safety Evaluations: Experts identify fall risks and improve safety in the home environment.
- Ongoing Care Coordination: Regular monthly check-ins ensure medication management and health updates are shared with primary care providers or neurologists.
- Caregiver Support Services: Education, training, and counseling to help caregivers handle dementia-related challenges.
- Access to Community Resources: Connecting families with local organizations and support systems.
- In-Home Respite Care: Up to $2,500 per year for eligible participants to receive relief through professional dementia home care services.
How to Qualify
To participate, individuals must be enrolled in Medicare Part B and be evaluated by an approved GUIDE provider. Once diagnosed with dementia and deemed eligible, the patient is officially enrolled and begins receiving benefits, including in-home care and caregiver support.
