Scope of Care
If we had the power to reverse the effects of aging, obliterate chronic diseases or wipe away long-term disabilities, we would. What we can do is help you or an aging loved one live your best life possible. From 40 to 104, we can assist adults of all ages with a variety of everyday activities. We offer four levels of care:
1. Companion Care Services
To help our clients lead a more independent lifestyle, our caregivers can assist with:
- Light housekeeping
- Outdoor/indoor walks
- Meal preparation
- Transportation to doctor appointments, shopping centers or assistance with other errands
2. Personal Care Services
For those who need more direct care with daily living activities, our caregivers assist with:
- Transfers using gait belts
- Hospice care support
- Ambulation
- Bathing, showering and dressing
- Alzheimer’s and other forms of dementia care
3. Medication Care Services
Under the supervision of a registered nurse (RN), our caregivers are trained to help prevent:
- Failure to initially fill a prescription or refill as directed
- Taking more or less of a medication than prescribed
- Taking a dose at the wrong time
- Taking expired or damaged medication
Our licensing allows our caregivers, under the direction of our RN, to directly assist with our clients’ medications or administer medications.
4. Nursing Care Services
Our RNs create custom care plans that follow the directives of our clients’ doctors. Our caregivers can assist with:
- Nurse Delegations
- Transitions of care from hospital to home
- Diabetes self-management
- Care for chronic diseases and illnesses
- Long-term disability care
