But our team offers constant contact with the discharge planners, following discharge recommendations, which not only takes the anxiety away from your loved one’s mind but also helps to build trust and confidence. Additionally, we assist with meal prep, 24-hour care, transportation, medication reminders, and light housekeeping to further add to our holistic care approach.
